How to request social care help and support, including assessments.

Request a social care assessment

To support you in understanding what support is available to meet your needs, we will require you to have a social work assessment. 

You may first want to read our eligibility criteria and read our charging policy.

What happens during an assessment

Our customer service team will ask you a number of simple questions and ask you to complete an initial screening form to help understand your care needs. This could include questions around:

  • your medical history, and any medical conditions or disabilities that affect your ability to live independently
  • how you prepare and cook food
  • how you manage everyday activities, such as getting out of bed, washing, dressing and going to the toilet
  • how you manage your life at home, such as shopping, laundry, housework and any general household maintenance
  • how you make decisions, and whether there is anything you feel would help you to make decisions about your life

They will also:

  • explain more about our eligibility criteria and charging policy
  • refer you for a more detailed assessment of your personal needs, if it is thought that you are eligible for a service

If you're not eligible

If you're not eligible for support, they will try to give you as much information as they can about alternative sources of support, if appropriate.
